What Should a Prescription Contain?

Doctor’s Identity: Full name, clinic/hospital name, and contact details on official letterhead.

Medical Registration Number: Valid registration (e.g., MD/DNB) and license number as issued by the Medical Council of India.

Date of Issue: The exact date when the prescription was written.

Patient Information: Patient’s full name and date of birth.

Medication Details: Drug name (generic and brand), strength (e.g,. 200 mg), total quantity, and dosage instructions.

Doctor’s Authorization: Hand‑signed by the prescribing physician and stamped with their official seal.

Official Prescription Slip: Written on the treating doctor’s own prescription pad—no photocopies or handwritten notes on plain paper
