FDA Approves Jaypirca for Previously Untreated CLL and SLL

Nitin Goswami October 23, 2026

Quick answer: On 2 October 2026, the US FDA approved Jaypirca (pirtobrutinib, Eli Lilly) as a first treatment for adults with chronic lymphocytic leukemia (CLL) or small lymphocytic lymphoma (SLL) with no known 17p deletion. In the BRUIN CLL-313 trial of 282 people, pirtobrutinib cut the risk of the disease getting worse or death by 80% compared with bendamustine plus rituximab chemo-immunotherapy.

A Once-a-Day Tablet Instead of Chemotherapy as the First Step

CLL is found through blood tests that show too many lymphocytes, a type of white blood cell. The US National Cancer Institute describes it as a cancer of the blood and bone marrow that usually gets worse slowly, and some people are first managed with watchful waiting instead of treatment. When treatment is needed, though, the first choice matters a lot.

Until now, Jaypirca was approved only for people whose CLL had come back after earlier treatment. This approval moves it to the very start. For a newly diagnosed patient whose doctor says "it's time to treat", that means one more tablet-based option to talk about. In India, a doctor can request it for a patient through a Named Patient Program.

How Pirtobrutinib Works

In CLL, the cancer cells are B-cells, and they rely on a signalling protein called BTK (Bruton's tyrosine kinase) for growth, movement and attachment. Pirtobrutinib blocks BTK.

What makes it different is how it attaches. The label calls it a noncovalent BTK inhibitor and says it binds both normal BTK and BTK that carries C481 mutations. Its earlier CLL approval was for people whose disease came back after a covalent BTK inhibitor, the older type of drug in this class.

One point that often confuses families: CLL is a chronic leukemia and is not the same disease as acute lymphoblastic leukemia, even though both start in lymphocytes. The treatments are different, so information about one does not carry over to the other.

What the BRUIN CLL-313 Trial Showed

The study enrolled 282 adults with untreated CLL or SLL who did not have a 17p deletion. Half took pirtobrutinib every day. The other half got six cycles of bendamustine plus rituximab (often called BR), a chemo-immunotherapy combination.

Keeping the disease in check

  • After a median follow-up of 28 months, the median progression-free survival with pirtobrutinib had not yet been reached, meaning more than half the patients were still free of progression
  • With BR, the median progression-free survival was 33.5 months
  • Hazard ratio 0.20: an 80% lower risk of progression or death with pirtobrutinib

Survival so far

It is too early for a final answer on overall survival. At the time of the analysis, there had been 13 deaths in total: 3 (2.1%) in the pirtobrutinib group and 10 (7.1%) in the BR group. The FDA notes the survival data are still immature, so these numbers should be read with care.

Side Effects Seen in the Trial

Most common

  • Upper respiratory tract infections (27%)
  • Rash (22%)
  • COVID-19 (21%)
  • The most common serious lab change was a low neutrophil count, a type of infection-fighting white cell

Serious side effects happened in 28% of people taking pirtobrutinib.

Warnings in the prescribing information

  • Infections
  • Bleeding (haemorrhage)
  • Low blood counts (cytopenias)
  • Heart rhythm problems (cardiac arrhythmias)
  • New, second cancers
  • Liver injury (hepatotoxicity)
  • Harm to an unborn baby

Tell your doctor about any blood thinners you take, any heart rhythm history, and any fever or unusual bruising during treatment.

What "No Known 17p Deletion" Means for You

17p deletion is a change in the CLL cells where part of chromosome 17 is missing. The trial did not include people with this change, so the new approval does not cover them. Ask your haematologist whether your CLL has been tested for 17p deletion, and keep a copy of the report, because it decides whether this approval applies to you.

Jaypirca's Approvals Now

According to the updated US label (October 2026), Jaypirca is approved for adults with:

  • Previously untreated CLL/SLL with no known 17p deletion (new)
  • CLL/SLL that has come back or stopped responding after a covalent BTK inhibitor
  • Mantle cell lymphoma that has come back after at least two earlier treatments, including a BTK inhibitor (accelerated approval)

Doctors and families who want the full product details, storage and import steps can see the Jaypirca medicine page.

Frequently Asked Questions

Is Jaypirca chemotherapy?

No. It is a targeted tablet that blocks the BTK protein. In this trial it was compared against a chemotherapy-based combination (bendamustine plus rituximab).

How is Jaypirca taken?

The recommended dose is 200 mg once a day by mouth, swallowed whole with water, with or without food. The tablets should not be cut, crushed or chewed.

Can Jaypirca be used if I have a 17p deletion?

This new first-line approval is only for CLL/SLL with no known 17p deletion. Your haematologist will discuss other options if your test shows the deletion.

Is Jaypirca available in India?

Jaypirca can be imported for an individual patient through a regulated Named Patient Program, once the treating haematologist or oncologist prescribes it. We help with the paperwork and the import permission.

Why not buy it from any online seller?

Tablets from unregulated sellers can be fake, expired or stored badly, and they often look exactly like the real product. Through NPP, the medicine comes from verified sources with proper import documents, so you know what you are taking.
