What Is Elahere Used For?

Elahere (mirvetuximab soravtansine-gynx) is a first-in-class antibody-drug conjugate (ADC) made up of an antibody that binds folate receptor alpha (FRα), a cleavable linker, and the maytansinoid payload DM4 — a potent tubulin inhibitor. FRα is overexpressed on the surface of many ovarian cancer cells but is limited in healthy tissue. The antibody attaches to FRα-positive tumour cells, the ADC is internalised, and DM4 is released inside the cell to disrupt microtubules and trigger cancer cell death, delivering chemotherapy more selectively to the tumour. Elahere is indicated:
  • for the treatment of adult patients with folate receptor alpha (FRα)-positive, platinum-resistant epithelial ovarian, fallopian tube, or primary peritoneal cancer, who have received one to three prior systemic treatment regimens. Patients are selected for therapy based on an approved test.
The medicine first received US FDA accelerated approval in November 2022 based on the SORAYA trial, and was converted to full (regular) approval on 22 March 2024 based on the Phase III MIRASOL trial. In MIRASOL, Elahere reduced the risk of disease progression or death by 35% and the risk of death by 33% compared with investigator's choice single-agent chemotherapy — making it the first therapy to demonstrate an overall survival benefit in a Phase III trial in platinum-resistant ovarian cancer. In November 2024, the European Commission granted marketing authorisation for Elahere in adult patients with FRα-positive, platinum-resistant high-grade serous epithelial ovarian, fallopian tube or primary peritoneal cancer who have received one to three prior systemic treatment regimens, making it the first and only FRα-directed ADC approved in the European Union.

Important: Elahere is a biomarker-directed therapy. Eligibility requires confirmation of FRα positivity on tumour tissue using an approved companion diagnostic (the VENTANA FOLR1 (FOLR1-2.1) RxDx Assay). It is a prescription anticancer medicine and must be used under the supervision of a qualified gynaecologic or medical oncologist.

Recommended Dosage:

The recommended dosage of Elahere is 6 mg/kg based on adjusted ideal body weight (AIBW), administered as an intravenous (IV) infusion once every 3 weeks (a 21-day cycle), continued until disease progression or unacceptable toxicity. The dose is not calculated on actual body weight, and the maximum recommended dose should not be exceeded.

Elahere must be diluted in 5% dextrose injection before administration and given through an infusion line with an in-line filter, as described in the prescribing information. The first infusion is generally given over a longer duration with close monitoring for infusion-related reactions, and patients are observed for at least one hour after the first infusion; subsequent infusions may be given more rapidly if earlier infusions were well tolerated.

Premedication before each infusion typically includes a corticosteroid, an antihistamine, an antipyretic, and an antiemetic to reduce infusion-related reactions and nausea.

Doses may be delayed, reduced, or permanently discontinued for ocular toxicity, pneumonitis, peripheral neuropathy, or other adverse reactions in line with the dose-modification tables in the full prescribing information.

Elahere Warnings and Precautions

  • Elahere may cause serious eye problems, including blurred vision, dry eyes, light sensitivity, and corneal changes. Use prescribed eye drops, attend regular eye exams, report vision changes immediately, and avoid contact lenses unless advised by your doctor.
  • Treatment with mirvetuximab soravtansine-gynx may cause serious or life-threatening lung inflammation. Contact your healthcare provider immediately if you develop a new or worsening cough, shortness of breath, or chest discomfort. Treatment may be delayed or permanently stopped if severe pneumonitis occurs.
  • This therapeutic drug may cause nerve damage, leading to numbness, tingling, burning, pain, or weakness in your hands and feet. Report these symptoms promptly, as your doctor may adjust the dose or temporarily stop treatment.
  • Infusion-related reactions, including fever, chills, flushing, breathing difficulty, or low blood pressure, may occur during treatment. You will be monitored closely, and the infusion may be slowed, paused, or stopped if needed.
  • This medicine may harm an unborn baby. Use effective contraception during treatment and for the recommended period afterward. Do not breastfeed during treatment or until your doctor advises it is safe after the last dose.
  • Elahere therapy may cause blurred vision, tiredness, or reduced alertness. Avoid driving, operating machinery, or performing activities requiring clear vision until your doctor confirms it is safe to do so.
  • This medicinal product may increase liver enzymes and reduce blood counts, albumin, or magnesium levels. Your healthcare provider will perform blood tests before each treatment to monitor your health and manage any side effects early.
