Braftovi (encorafenib) is an oral kinase inhibitor that targets BRAF V600E and related mutant BRAF kinases in the MAPK signalling pathway. Mutations in the BRAF gene, such as BRAF V600E, produce constitutively activated BRAF kinases that continuously stimulate tumour cell growth. By inhibiting these kinases, Braftovi interrupts the growth signal and slows tumour proliferation. It is almost always used in combination with another agent — binimetinib (Mektovi®) or cetuximab (Erbitux®) with chemotherapy — depending on the tumour type. Braftovi is indicated for:
Limitations of Use: Braftovi is not indicated for the treatment of patients with wild-type BRAF melanoma, wild-type BRAF colorectal cancer, or wild-type BRAF NSCLC.
Important: Braftovi is a biomarker-directed therapy. Eligibility requires laboratory confirmation of a BRAF V600E (or, in melanoma, V600K) mutation. It is a prescription anticancer medicine and must be used under the supervision of a qualified medical oncologist.
Braftovi is taken orally once daily, at approximately the same time each day, with or without food. The capsules should be swallowed whole and should not be opened, crushed or chewed. The dose depends on the indication and the combination partner:
For adverse reactions, the dose may be reduced in defined steps, or treatment interrupted or permanently discontinued, as set out in the prescribing information. A missed dose should not be taken within 12 hours of the next scheduled dose. If vomiting occurs after taking Braftovi, no additional dose should be taken — treatment should continue at the next scheduled dose. Dose adjustments are also required with strong or moderate CYP3A4 inhibitors, and concomitant strong CYP3A4 inducers should be avoided.
