Welireg (belzutifan) is an oral hypoxia-inducible factor 2 alpha (HIF-2α) inhibitor. In cells affected by von Hippel-Lindau (VHL) disease and certain kidney cancers, HIF-2α accumulates and drives tumor growth by activating genes involved in angiogenesis and cell proliferation. Belzutifan blocks this pathway, helping slow or stop tumor growth. It is indicated for the treatment of:

  • Adult patients with von Hippel-Lindau (VHL) disease who require therapy for associated renal cell carcinoma (RCC), central nervous system (CNS) hemangioblastomas, or pancreatic neuroendocrine tumors (pNET), not requiring immediate surgery.
  • Adult patients with advanced renal cell carcinoma (RCC) following a PD-1 or PD-L1 inhibitor (such as Opdivo) and a VEGF tyrosine kinase inhibitor (VEGF-TKI).
  • Adult and pediatric patients 12 years and older with locally advanced, unresectable, or metastatic pheochromocytoma or paraganglioma (PPGL).
  • Adult patients with renal cell carcinoma with a clear cell component (ccRCC), in combination with pembrolizumab, as adjuvant therapy following nephrectomy in patients at intermediate-high or high risk of recurrence.

Welireg is a prescription anticancer medicine and must be initiated and supervised by a qualified medical oncologist.

Recommended Dosage: The recommended dose of Welireg is 120 mg (three 40 mg tablets) taken orally once daily, with or without food, at approximately the same time each day. For patients weighing less than 40 kg with pheochromocytoma or paraganglioma, the recommended dose is 80 mg (two 40 mg tablets) once daily. Tablets should be swallowed whole and should not be chewed, crushed, or split. Dose modification or interruption may be required in cases of anemia, hypoxia, or other treatment-related toxicities, as determined by the treating physician.

Warnings and Precautions for Welireg

Exposure to Welireg during pregnancy can cause harm to an unborn baby. Pregnancy status must be verified before starting treatment, and patients need to use effective non-hormonal contraception, as Welireg can make some hormonal contraceptives ineffective.

Coadministration with inhibitors of UGT2B17 or CYP2C19 can increase belzutifan levels and the risk of side effects.
