Blincyto (blinatumomab) is a bispecific CD19-directed CD3 T-cell engager (BiTE®) immunotherapy. It is designed to bridge the patient's own T cells (which carry the CD3 protein) to leukemia cells (which carry the CD19 protein on B cells), bringing the T cells close enough to recognize and destroy the malignant B cells. It is indicated for the treatment of adult and pediatric patients one month and older with:
CD19-positive B-cell precursor acute lymphoblastic leukemia (ALL) in first or second complete remission with minimal residual disease (MRD) greater than or equal to 0.1%.
Relapsed or refractory CD19-positive B-cell precursor acute lymphoblastic leukemia (ALL).
CD19-positive Philadelphia chromosome-negative B-cell precursor acute lymphoblastic leukemia (ALL)
in the consolidation phase of multiphase chemotherapy.
It is a prescription anticancer medicine and must be used under the supervision of a qualified hematologist-oncologist.
The dose is weight-based: patients weighing at least 45 kg receive a fixed (flat) dose, while patients weighing less than 45 kg receive a dose calculated by body surface area (BSA). To reduce the risk of cytokine release syndrome and neurological events, a lower starting dose (dose step) is often used at the beginning of the first cycle and then increased.

Blincyto may cause Tumor Lysis Syndrome (TLS), a serious condition caused by the rapid breakdown of cancer cells. Your doctor may recommend extra fluids, medicines, and regular monitoring to help reduce the risk of this complication.
Some patients may experience reactions during or shortly after the Blincyto infusion, such as fever, chills, breathing difficulty, or low blood pressure. Healthcare professionals will monitor you closely and provide treatment if these reactions occur.
Blincyto may cause side effects such as confusion, dizziness, tremors, or problems with coordination. Because of these effects, avoid driving, operating heavy machinery, or performing activities that require full alertness until your doctor advises it is safe.
Blincyto may harm an unborn baby. Women should use effective contraception during treatment and for the recommended period after the last dose. Breastfeeding is not recommended during treatment. Always discuss pregnancy or breastfeeding plans with your doctor.
