Truqap Uses and Indications

Truqap (capivasertib) is a targeted oral anticancer medicine known as an AKT inhibitor. AKT is a signalling protein inside cells that helps control cell growth, survival, and metabolism. In certain cancers, the PI3K/AKT/PTEN pathway becomes overactive — often because of genetic alterations in PIK3CA, AKT1, or loss of PTEN — which allows tumour cells to keep growing. Capivasertib blocks all three AKT isoforms (AKT1, AKT2, AKT3), switching off this cancer-driving signal. Because its activity depends on the tumour's biomarker profile, patients must be selected using an approved diagnostic test.

Truqap is indicated for:


  • HR-positive, HER2-negative locally advanced or metastatic breast cancer in adults with one or more PIK3CA, AKT1, or PTEN alterations (as detected by an approved test), following progression on at least one endocrine-based regimen in the metastatic setting or recurrence on or within 12 months of completing adjuvant therapy — used in combination with fulvestrant.
  • PTEN-deficient metastatic androgen pathway modulation-naïve or -sensitive (mAPMN/S) prostate cancer — previously referred to as metastatic hormone-sensitive prostate cancer (mHSPC) — where PTEN deficiency is confirmed by an authorized companion diagnostic test (VENTANA PTEN SP218 RxDx Assay), used in combination with abiraterone and prednisone.
Truqap is a prescription anticancer medicine and must be used only under the supervision of a qualified medical oncologist or uro-oncologist.

Recommended Dosage:

The recommended starting dose is 400 mg twice daily (approximately 12 hours apart) — usually given as two 200 mg tablets per dose. Treatment is continued until disease progression or unacceptable toxicity. Truqap is taken orally, with or without food, on an intermittent weekly schedule: 4 consecutive days on treatment, followed by 3 days off, repeated each week. 

Tablets should be swallowed whole. Do not chew, crush, or split them, and do not take tablets that are broken, cracked, or otherwise not intact. Where a dose reduction is required for side effects, the recommended reduced dose levels are 320 mg twice daily and then 240 mg twice daily. Because capivasertib is a CYP3A substrate, concomitant use with strong CYP3A inhibitors should be avoided; if unavoidable, or when a moderate CYP3A inhibitor is used, the dose is reduced to 320 mg twice daily on the same 4-days-on/3-days-off schedule.

Fasting glucose and HbA1c should be evaluated before starting Truqap and at regular intervals during treatment. Pregnancy status must be verified in females of reproductive potential before initiating therapy.

Truqap Warnings and Precautions

  • Truqap can cause severe hyperglycemia, including diabetic ketoacidosis, which may rarely be fatal. Your doctor will monitor blood sugar before and during treatment. Report excessive thirst, frequent urination, confusion, unusual tiredness, or fruity-smelling breath immediately, especially if you have diabetes or pre-diabetes.
  • Diarrhoea is very common with Truqap and may become severe. Begin anti-diarrhoeal treatment as advised at the first sign of loose stools, drink plenty of fluids, and contact your doctor if symptoms persist or worsen, as treatment adjustments may be required.
  • Truqap may cause serious skin reactions, including Stevens-Johnson syndrome and erythema multiforme. Seek immediate medical attention if you develop rash, blistering, peeling skin, mouth or eye sores, or rash accompanied by fever during treatment with Truqap.
  • Do not use Truqap if you have a known severe allergy to capivasertib or any of its ingredients. Inform your doctor immediately if you experience swelling, difficulty breathing, severe rash, or any signs of an allergic reaction after taking Truqap.
  • Truqap may harm an unborn baby. Use effective contraception during treatment and for the recommended period after the last dose. Do not breastfeed during treatment. Discuss pregnancy or breastfeeding plans with your doctor before starting Truqap therapy.
  • Truqap may cause fatigue, nausea, vomiting, dizziness, and mouth sores. If you feel tired, dizzy, or generally unwell, avoid driving, operating machinery, or performing tasks requiring alertness until your doctor confirms it is safe.
  • Do not start, stop, or change any prescription medicines, over-the-counter products, supplements, or herbal remedies without consulting your doctor. Certain medicines, especially CYP3A inhibitors and inducers, can affect Truqap levels and may require dose adjustments.

TRUQAP (capivasertib) tablets may be imported in the patient's name only, by patients or government hospitals. The following documentation is required:
