What Is Bireociclib Used For?

Bireociclib is an oral tablet, taken twice a day, for advanced breast cancer that is hormone receptor-positive and HER2-negative (HR+/HER2−). It was discovered and developed in China by Xuanzhu Biopharmaceutical, part of the Sihuan Pharmaceutical group, and is sold there as 轩悦宁® (Xuanyuening®). Its Chinese generic name is 吡洛西利片.

China's National Medical Products Administration (NMPA) first approved it in May 2025 and added a first-line use in February 2026. It is now approved for adults with HR+/HER2− locally advanced or metastatic breast cancer:

  • First treatment: together with an aromatase inhibitor, as the initial hormone (endocrine) therapy in women
  • After hormone therapy has stopped working: together with fulvestrant
  • On its own: when the cancer has grown after two or more hormone therapies and one chemotherapy in the metastatic setting

That last use is unusual: in China it is the only medicine of its class approved to be taken alone for advanced breast cancer. It is a prescription cancer medicine and should only be used under a medical oncologist.

How It Works

Breast cancer cells rely on proteins called cyclin-dependent kinases (CDKs) to move through the cell cycle and divide. Bireociclib blocks CDK4 and CDK6, and also has some effect on CDK2 and CDK9. The company designed it to hit CDK4 much harder than CDK6 (about 18 times more selective), because strong CDK6 blockade is linked to low white blood cell counts. Paired with hormone therapy, it helps hold back tumour growth for longer.

What the Trials Showed

  • BRIGHT-2 (with fulvestrant, phase 3, 305 patients): the time before the cancer grew was roughly doubled, a median of 14.69 months against 7.33 months with placebo plus fulvestrant, as judged by the doctors (HR 0.542). An independent review found 17.51 vs 7.29 months. Tumours shrank in 45.6% of patients compared with 14.9%.
  • BRIGHT-3 (first-line, with an aromatase inhibitor, phase 3): 397 patients were enrolled. At the interim analysis it cut the risk of the cancer growing or death by 47% (HR 0.53); median progression-free time had not yet been reached, against 18.43 months in the placebo group. 63.5% responded, compared with 42.5%.
  • BRIGHT-1 (alone, phase 2, 131 heavily pre-treated patients): 29.8% responded, the median time before the cancer grew was 11.0 months, and median survival was 29.0 months.

Recommended Dosage

According to the Chinese prescribing information:

  • With hormone therapy (an aromatase inhibitor or fulvestrant): 360 mg (three 120 mg tablets) twice a day, in 28-day cycles
  • Fulvestrant, when used: 500 mg injection on days 1, 15 and 29, then once a month
  • On its own: 480 mg (four tablets) twice a day, in 28-day cycles
  • It is taken every day without a break, and continued as long as it is working and side effects are manageable

Because there is only one tablet strength, the dose is lowered simply by taking fewer tablets. Your oncologist sets the exact plan.

This information is not a treatment plan. For personalized medical advice, consult your doctor and refer to the full prescribing information.

Warnings and Precautions

Diarrhoea: this is the most common side effect. In BRIGHT-2 it affected 92.6% of patients on bireociclib with fulvestrant, usually in the first cycle and mostly mild. The company reports severe (grade 3–4) diarrhoea in 6.4%, with no grade 4 cases. Keep anti-diarrhoea medicine at home, drink plenty of fluids and tell your doctor early.

  • Low blood counts: neutropenia (89.2%) and low white cells (86.8%) were very common in BRIGHT-2, along with anaemia. Blood tests are done regularly and the dose can be paused or reduced. Report fever or signs of infection straight away.
  • When taken alone: at the higher 480 mg dose in BRIGHT-1, diarrhoea (93.1%), low neutrophils (87.0%), low white cells (86.3%) and vomiting (78.6%) were the most common problems.
  • Other medicines: avoid strong CYP3A4 inhibitors and inducers. Give your doctor a full list of what you take.
  • Allergy: do not take it if you are allergic to bireociclib or any ingredient.
  • Partner medicines: the warnings for fulvestrant or the aromatase inhibitor also apply.

Overall: in BRIGHT-2, severe (grade 3 or higher) side effects occurred in 74.5% of patients on bireociclib and 19.8% on placebo, mainly blood count and gut effects. 42.2% needed a dose reduction, but only 4.4% stopped bireociclib because of side effects. There is no boxed warning on the Chinese label. Safety in pregnancy, breastfeeding and children has not been established.

Documentation & Availability

What documents are required to import Bireociclib to India?

The tablets can be imported only in the patient's name, by the patient or a government hospital, in line with India's import regulations for unapproved medicines. You will need:

  • A valid prescription from a qualified medical oncologist
  • Pathology reports confirming HR-positive, HER2-negative breast cancer, recent scans, and details of earlier hormone therapy or chemotherapy
  • Patient ID proof (issued by the Government of India)

How is the order confirmed?

Once we have received and checked a valid prescription and, where needed, an import permit.

Is Bireociclib available in India?

No. It is approved only in China. It has no CDSCO approval, it is not sold in Indian pharmacies, and the US FDA and European Medicines Agency have not approved it either.

Patients can still get it legally through the Named Patient Program (NPP), which allows a medicine not registered in India to be imported for one specific patient on a prescription. For other targeted options in HR-positive advanced breast cancer, see our pages on Orserdu and Truqap.

It can be made available to patients, doctors, and hospitals across major cities in India, including Gurgaon, Delhi, Bangalore, Mumbai, Navi Mumbai, Thane, Hyderabad, Mysore, Kerala, Chennai, Coimbatore, Madurai, Kolkata, Ahmedabad, Surat, Vadodara, Pune, Nagpur, Nashik, Jaipur, Udaipur, Lucknow, Kanpur, Gorakhpur, Chandigarh, Mohali, Faridabad, Noida, Ghaziabad, Kochi, Trivandrum, Kozhikode, Thrissur, Kannur, Mangalore, Bhubaneswar, Patna, Ranchi, Raipur, Guwahati, Visakhapatnam, Vijayawada, Goa, Jammu, Srinagar, Dehradun, Bhopal, Indore, Ludhiana, Amritsar, and others — and can facilitate delivery to the patient's treating hospital. Orders are confirmed only after receipt of a valid doctor's prescription and an import permit, where applicable.
