What Is Finotonlimab Used For?

Finotonlimab is an immunotherapy medicine developed and made in China by Sinocelltech, where it is sold as 安佑平® (An You Ping). It belongs to the PD-1 antibody family, which helps the immune system recognise and attack cancer cells. It is given as a drip every three weeks, always together with other cancer treatment.

China's National Medical Products Administration (NMPA) approved it in February 2025 for two cancers:

  • Head and neck squamous cell carcinoma: with platinum-based chemotherapy, as the first treatment for cancer that has come back (recurrent) and/or spread (metastatic)
  • Liver cancer (hepatocellular carcinoma): combined with bevacizumab, for unresectable or metastatic disease in patients who have not had any earlier systemic treatment

It is a prescription cancer medicine and should only be used under a medical oncologist.

How It Works

T cells, the immune system's fighters, carry a "brake" called PD-1. Many tumours press on this brake to switch the T cells off. Finotonlimab is a humanised IgG4 antibody that sits on PD-1 and blocks the tumour's signal, so the T cells can become active against the cancer again. Sinocelltech notes that its IgG4 design has low ADCC and CDC activity, meaning it is built not to destroy the immune cells it binds to.

What the Trials Showed

Head and neck cancer: the phase 3 trial published in Nature Medicine in 2024 included 370 patients in China. Adding finotonlimab to cisplatin and 5-fluorouracil chemotherapy extended median survival from 10.5 to 14.1 months (hazard ratio 0.73). At one year, 53.5% of patients were alive against 39.4%. Responses also lasted far longer, a median of 19.3 months against 5.0 months. In patients whose tumours had a PD-L1 score (CPS) of 20 or more, median survival reached 20.1 months.

Liver cancer: in a phase 3 trial of 346 patients presented at ASCO 2024, finotonlimab with a bevacizumab biosimilar was compared with a standard tablet treatment. Median survival was 22.1 months against 14.2 months (hazard ratio 0.60), and the time before the cancer grew was 7.1 against 2.9 months. About a third of patients (32.8%) responded, compared with 4.3%.

Recommended Dosage

In both phase 3 trials the dose was:

  • Finotonlimab: 200 mg (two 100 mg vials) as an intravenous drip on day 1 of each 3-week cycle
  • Head and neck cancer: given with cisplatin 75 mg/m² on day 1 and 5-fluorouracil 750 mg/m² on days 1 to 5 for up to 6 cycles, then finotonlimab alone as maintenance, for up to 2 years in total
  • Liver cancer: given with bevacizumab 15 mg/kg every 3 weeks, for as long as it is helping

Chemotherapy is given about an hour after the finotonlimab drip. Your oncologist sets the exact plan and the number of cycles.

Warnings and Precautions

Like other PD-1 medicines, finotonlimab can make the immune system attack healthy organs. The figures below are from the head and neck cancer trial, where it was given with chemotherapy.

  • Immune-related side effects: 38.9% of patients had one, against 22.0% on chemotherapy alone. Most were mild (grade 1–2). The most common were an underactive thyroid (17.8%) and skin inflammation (10.9%).
  • Thyroid: underactive thyroid (19.8% vs 11.4%) and overactive thyroid (7.3% vs 2.4%) were more frequent. Thyroid blood tests are done during treatment.
  • Other organs: immune inflammation can also affect the lungs, liver, bowel, kidneys or hormone glands. Report a new cough, breathlessness, diarrhoea, yellow skin or severe tiredness quickly.
  • Low potassium and appetite loss: low potassium (8.5% vs 2.4%), nausea and reduced appetite were a little more common.
  • Infusion reactions: none were grade 3 or higher, and none led to stopping treatment.

Overall: severe (grade 3–5) treatment-related side effects occurred in 25.1% of patients on finotonlimab plus chemotherapy and 17.1% on chemotherapy alone, and 3.6% stopped because of side effects. In the liver cancer trial, severe treatment-related side effects occurred in 42.6% of patients on the combination, most often high blood pressure (7.8%), and three deaths from bleeding or unknown causes were linked to the bevacizumab partner. Safety in pregnancy, breastfeeding and children has not been established.
