Zepbound (tirzepatide) is a once-weekly injectable medicine approved by the FDA, in combination with a reduced-calorie diet and increased physical activity, to reduce excess body weight and maintain weight reduction in adults with obesity or overweight with at least one weight-related condition, and to treat moderate to severe obstructive sleep apnea (OSA) in adults with obesity.
Zepbound is a dual GIP and GLP-1 receptor agonist. By activating both of these gut hormone receptors, it helps reduce appetite and food intake, slow gastric emptying, and improve metabolic signaling, leading to significant weight reduction in clinical studies.
Zepbound is intended for adults with obesity, or overweight with at least one weight-related comorbid condition, seeking chronic weight management, as well as adults with obesity and moderate to severe obstructive sleep apnea. Note: Zepbound shares the same active ingredient as Mounjaro, which is approved for a different indication — type 2 diabetes.
Zepbound is started at 2.5 mg subcutaneously once weekly for 4 weeks, then increased in 2.5 mg increments at intervals of at least 4 weeks based on response and tolerability, up to a maintenance dose of 5 mg to 15 mg once weekly.
Boxed Warning — Zepbound: In studies with rodents, tirzepatide caused thyroid C-cell tumors. It is unknown whether Zepbound causes thyroid C-cell tumors, including medullary thyroid carcinoma (MTC), in humans. Zepbound is contraindicated in patients with a personal or family history of MTC or in patients with Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).
Severe gastrointestinal adverse reactions have been reported, which in some cases have led to acute kidney injury or worsening of chronic kidney disease due to volume depletion.
Acute pancreatitis and acute gallbladder disease have been reported. Zepbound should be discontinued if pancreatitis is suspected.
The risk of hypoglycemia is increased when Zepbound is used with insulin or insulin secretagogues. Patients with a history of diabetic retinopathy should be monitored for progression.
Zepbound delays gastric emptying and may increase the risk of pulmonary aspiration during procedures requiring anesthesia or deep sedation. Serious hypersensitivity reactions have also been reported.
