What Is Bevifibatide Used For?

Bevifibatide is a short-acting, drip-fed blood thinner used during angioplasty for people with a heart attack or unstable angina. It was developed and is made in China by Bio-Thera Solutions, Ltd., and is sold there as 贝塔宁® (BETAGRIN®). It is given in the cath lab, around the time a narrowed heart artery is opened and stented, to stop clots forming on the fresh stent.

China's National Medical Products Administration (NMPA) approved it in 2024 (approval no. 国药准字H20240030) for:

  • Patients with acute coronary syndrome (ACS) undergoing percutaneous coronary intervention (PCI), including coronary stent placement
  • To lower the risk of sudden re-blockage of the artery, clot inside the stent, and "no-reflow" or "slow-flow" (when blood fails to run freely through the opened artery)

It is a hospital-only heart medicine and is given only by an interventional cardiology team.

How It Works

When an artery is ballooned and stented, platelets rush to the damaged spot and stick together. They do this through a receptor on their surface called GPIIb/IIIa (also known as αIIbβ3). Bevifibatide is a small ring-shaped peptide that sits on this receptor and stops fibrinogen and von Willebrand factor from binding, so platelets cannot clump into a clot. According to the Chinese label, it also blocks a related receptor, αvβ3, found on cells in the artery wall. In studies it blocked more than 80% of platelet clumping soon after the bolus, and platelet function returned quickly once the drip was stopped.

What the Trial Showed

The approval rests on the phase 3 BES-PCI trial in China, which included 1,442 people with ACS having PCI (721 on bevifibatide and 721 on placebo, all also on standard antiplatelet treatment). Within 30 days, the combined rate of death, heart attack, urgent repeat procedure on the same artery, need for rescue anti-clot treatment, and no-reflow or severe slow-flow was 3.47% with bevifibatide against 5.83% with placebo. That is a 43% lower relative risk (hazard ratio 0.57, 95% CI 0.35–0.94, p = 0.026). The need for rescue anti-clot treatment fell from 1.66% to 0.28%.

Recommended Dosage

The Chinese label sets the dose by body weight:

  • Bolus: 220 micrograms per kg (0.11 mL/kg), pushed into a vein over 1 to 2 minutes before PCI
  • Infusion: then 2.5 micrograms per kg per minute (0.075 mL/kg/hour) for about 24 hours, continued until the procedure is finished if it runs longer
  • Given with: aspirin and clopidogrel tablets before and after PCI, and heparin as the cardiologist decides, as set out on the label

Each vial holds 20 mg in 10 mL. The bolus is drawn up undiluted; the infusion dose is diluted in saline to 240 mL and run at 10 mL per hour. As an example, from the label's dosing table a 70 kg adult needs 7.5 mL for the bolus and 122.4 mL for the infusion, or about 13 vials in total. Your cardiologist sets the exact plan.

Warnings and Precautions

Bleeding: this is the most common problem, as with any medicine that stops platelets sticking. In the phase 3 trial, bleeding of any kind occurred in 7.88% of people on bevifibatide against 4.57% on placebo. Most of it was minimal (5.39%) or minor (2.21%), mainly from the gums, the puncture site, the gut or the urinary tract. Major bleeding occurred in 0.28% (2 people), and one person (0.14%) had a bleed in the brain. No one needed a transfusion for bleeding linked to the medicine. If bleeding cannot be controlled with pressure, the drip is stopped straight away.

  • Low platelets: 3 people (0.41%) had a fall in platelet count, all mild and all recovered. The drip should be stopped if the count drops sharply or below 100,000/mm³.
  • Kidneys: the medicine leaves the body mainly through the kidneys. There is no safety data in kidney impairment, and it must not be used in people on dialysis.
  • Other blood thinners: it must not be combined with another GPIIb/IIIa blocker, and use with clot-dissolving (thrombolytic) drugs is not recommended.
  • Tests before the drip: blood count, platelets, creatinine and clotting times (PT/aPTT), plus ACT during PCI.

Who should not receive it: people with a bleeding tendency or active bleeding in the past 30 days; very high blood pressure that is not controlled (above 200/110 mmHg); major surgery in the past 6 weeks; a past bleeding stroke, or any stroke in the past 30 days; a brain tumour, aneurysm or AV malformation; platelets below 100,000/mm³; a clotting disorder (INR above 2); severe liver failure; or a known allergy to any ingredient.

In the trial, 1.24% stopped treatment because of side effects, against 1.11% on placebo. Older patients had more bleeding than younger ones, but no dose change is needed. It has not been properly studied in pregnancy, and safety in children has not been established.

Documentation & Availability

What documents are required to import Bevifibatide to India?

The injection can be imported only in the patient's name, by the patient or a government hospital, in line with India's import regulations for unapproved medicines. You will need:

  • A valid prescription from a qualified cardiologist or interventional cardiologist
  • Medical reports confirming acute coronary syndrome and the planned or ongoing PCI, with the patient's body weight
  • Patient ID proof (issued by the Government of India)

How is the order confirmed?

Once we have received and checked a valid prescription and, where needed, an import permit.

Is Bevifibatide available in India?

No. It is approved only in China. It has no CDSCO approval, it is not sold in Indian pharmacies, and the US FDA and European Medicines Agency have not approved it either.

Patients can still get it legally through the Named Patient Program (NPP), which allows a medicine not registered in India to be imported for one specific patient on a prescription. Because it is used around a planned procedure, the hospital team should contact us well before the PCI date so the import can be completed in time.

It can be made available to patients, doctors, and hospitals across major cities in India, including Gurgaon, Delhi, Bangalore, Mumbai, Navi Mumbai, Thane, Hyderabad, Mysore, Kerala, Chennai, Coimbatore, Madurai, Kolkata, Ahmedabad, Surat, Vadodara, Pune, Nagpur, Nashik, Jaipur, Udaipur, Lucknow, Kanpur, Gorakhpur, Chandigarh, Mohali, Faridabad, Noida, Ghaziabad, Kochi, Trivandrum, Kozhikode, Thrissur, Kannur, Mangalore, Bhubaneswar, Patna, Ranchi, Raipur, Guwahati, Visakhapatnam, Vijayawada, Goa, Jammu, Srinagar, Dehradun, Bhopal, Indore, Ludhiana, Amritsar, and others — and can facilitate delivery to the patient's treating hospital. Orders are confirmed only after receipt of a valid doctor's prescription and an import permit, where applicable.

Sourcing & Delivery

NamedPatientProgram.com (NPP) sources this medicine through the manufacturer's authorised supply channels in China and arranges supply to patients, doctors, and hospitals under applicable legal provisions.
