What Is Cetagliptin Used For?

Cetagliptin is a once-daily tablet for type 2 diabetes, discovered and developed in China by CGeneTech (Suzhou), known in Chinese as 盛世泰科. Its Chinese name is 磷酸森格列汀片 (cetagliptin phosphate tablets) and its brand name is 盛捷维®; during development it was called 盛格列汀 and CGT-8012. It belongs to the DPP-4 inhibitor family of diabetes medicines, but has its own new chemical structure.

China's National Medical Products Administration (NMPA) approved it on 1 December 2024 as a Class 1 innovative drug (approval number 国药准字 H20240043), to improve blood sugar control in adults with type 2 diabetes:

  • On its own, together with diet and exercise
  • With metformin, when metformin alone is not controlling blood sugar well enough

It is a prescription endocrinology medicine and should be used under a diabetologist or endocrinologist.

How It Works

After a meal, the gut releases hormones (GLP-1 and GIP) that tell the pancreas to release insulin. An enzyme called DPP-4 breaks these hormones down quickly. Cetagliptin blocks DPP-4, so the hormones stay active for longer and the body releases more insulin when blood sugar is high. Because it works with the body's own signals, it did not cause drug-related low blood sugar in its trial on its own.

What the Trials Showed

Two phase 3 trials in China, both published in Diabetes, Obesity and Metabolism in 2023, compared it with placebo for 24 weeks, followed by 28 weeks of open treatment:

  • On its own (504 people): HbA1c fell by 1.08% on 50 mg and 1.07% on 100 mg, against 0.35% on placebo, a placebo-adjusted drop of 0.72%. 45.0% (50 mg) and 42.3% (100 mg) reached an HbA1c below 7.0%, compared with 12.9% on placebo.
  • Added to metformin (446 people): HbA1c fell by 1.23% on 50 mg and 1.17% on 100 mg. 51.1% and 49.4% reached an HbA1c below 7.0%, against 14.4% on metformin alone.

In both trials, people who started with a higher HbA1c (8.5% or more) saw bigger drops.

Recommended Dosage

According to the Chinese prescribing information:

  • Dose: one 50 mg tablet once a day, on its own or with metformin
  • When: preferably in the morning, with or without food
  • Kidneys: kidney function should be checked before starting and regularly after that. No dose change is needed for mild kidney impairment (eGFR 60 to 89 mL/min/1.73 m²); there are no data in moderate or severe kidney impairment, so it is not recommended for those patients for now
  • Liver: it has not been studied in people with liver impairment

The trials also tested 100 mg once a day, which did not lower HbA1c more than 50 mg, and the approved dose is 50 mg. Your doctor will decide whether to continue your other diabetes medicines.

This information is not a treatment plan. For personalized medical advice, consult your doctor and refer to the full prescribing information.

Warnings and Precautions

In the trials: side effects were no more common with cetagliptin than with placebo, in either trial. There was no drug-related low blood sugar when it was used on its own, and adding it to metformin did not increase the overall risk of side effects. CGeneTech also reports that, over the following 28 weeks, side effects on 100 mg occurred at a rate similar to placebo.

What the Prescribing Information Says

  • Side effects: those seen in at least 1% of people over 24 weeks, and more often than with placebo, were raised uric acid (2.7%), raised lipase (1.6%), raised triglycerides (1.3%) and raised ALT, a liver enzyme (1.1%). Across the two phase 3 trials there was no severe low blood sugar and no severe liver problem.
  • Who should not take it: anyone allergic to any ingredient of the tablet.
  • Pancreatitis: acute pancreatitis has been reported with other medicines of this class; none occurred in the pooled phase 3 trials of cetagliptin. Seek medical help for severe, lasting stomach pain.
  • Allergy: serious allergic reactions have been seen with other DPP-4 inhibitors after launch; none were seen in cetagliptin's trials.
  • Joint pain: severe and disabling joint pain has been reported with DPP-4 inhibitors; it was not seen in cetagliptin's trials.
  • Other medicines: no clinically important interaction was found with metformin. Interactions with other medicines have not been studied, so tell your doctor about everything you take.

Safety in pregnancy, breastfeeding and children has not been established in the published trials, so speak to your doctor first.

Documentation & Availability

What documents are required to import Cetagliptin to India?

The tablets can be imported only in the patient's name, by the patient or a government hospital, in line with India's import regulations for unapproved medicines. You will need:

  • A valid prescription from a qualified diabetologist, endocrinologist or physician
  • Recent reports confirming type 2 diabetes, such as HbA1c and fasting glucose, plus kidney and liver function tests
  • Patient ID proof (issued by the Government of India)

How is the order confirmed?

Once we have received and checked a valid prescription and, where needed, an import permit.

Is Cetagliptin available in India?

No. It is approved only in China. It does not appear in CDSCO's lists of new drugs approved in India, it is not sold in Indian pharmacies, and the US FDA and European Medicines Agency have not approved it either.

Patients can still get it legally through the Named Patient Program (NPP), which allows a medicine not registered in India to be imported for one specific patient on a prescription. As it is a daily tablet for a long-term condition, we usually plan supply a few months at a time.

It can be made available to patients, doctors, and hospitals across major cities in India, including Gurgaon, Delhi, Bangalore, Mumbai, Navi Mumbai, Thane, Hyderabad, Mysore, Kerala, Chennai, Coimbatore, Madurai, Kolkata, Ahmedabad, Surat, Vadodara, Pune, Nagpur, Nashik, Jaipur, Udaipur, Lucknow, Kanpur, Gorakhpur, Chandigarh, Mohali, Faridabad, Noida, Ghaziabad, Kochi, Trivandrum, Kozhikode, Thrissur, Kannur, Mangalore, Bhubaneswar, Patna, Ranchi, Raipur, Guwahati, Visakhapatnam, Vijayawada, Goa, Jammu, Srinagar, Dehradun, Bhopal, Indore, Ludhiana, Amritsar, and others — and can facilitate delivery to the patient's treating hospital. Orders are confirmed only after receipt of a valid doctor's prescription and an import permit, where applicable.
