Ecnoglutide is a once-weekly injection for type 2 diabetes and for long-term weight management. It was discovered and developed in China by Hangzhou Sciwind Biosciences, and in China it is sold as 先维盈® in a prefilled pen, with Pfizer China handling its commercial launch there.
China's National Medical Products Administration (NMPA) approved it in two steps:
It is a prescription endocrinology medicine and should be started and monitored by a diabetologist or endocrinologist.
After a meal, the gut releases a hormone called GLP-1. It helps the pancreas release insulin when blood sugar is high, slows down how fast the stomach empties and makes you feel full sooner. Ecnoglutide is a long-acting copy of this hormone. What makes it different is that it is "cAMP-biased": it switches on the main signalling route inside the cell while largely avoiding a second route (β-arrestin) that makes the receptor shut down over time. The company describes it as the first approved medicine of this biased type anywhere in the world.
Weight management (SLIMMER): this phase 3 trial, published in The Lancet Diabetes & Endocrinology in 2025, enrolled 664 Chinese adults with obesity or overweight who did not have diabetes. After 40 weeks, average body weight fell by 9.1%, 10.9% and 13.2% on the 1.2 mg, 1.8 mg and 2.4 mg doses, compared with no change (+0.1%) on placebo. Between 77% and 87% of people on the medicine lost at least 5% of their weight, against 16% on placebo. By week 48, the company reports a 15.4% average loss on the 2.4 mg dose (15.1% more than placebo).
Type 2 diabetes (EECOH-1): in 211 adults whose sugar was not controlled by diet and exercise alone or with a single oral tablet, HbA1c dropped by 1.96% on 0.6 mg and 2.43% on 1.2 mg after 24 weeks, compared with 0.87% on placebo. 80.3% of people on the 1.2 mg dose reached an HbA1c below 7%.
Type 2 diabetes with metformin (EECOH-2): in 621 adults already taking metformin, both doses lowered HbA1c by about 1.9% at 32 weeks and were at least as good as an established once-weekly comparison medicine (1.65%).
It is injected under the skin once a week. The pen sold in China holds 2.0 mg per mL in a 1.2 mL cartridge and lets the dose be adjusted from the starting dose up to the maintenance dose. In the trials, the dose was raised slowly to reduce stomach upset:
The exact schedule on the Chinese label should be followed, and your doctor will decide which dose is right for you.

Stomach and appetite effects: as with other medicines that act on the GLP-1 hormone, the most common side effects are digestive and mostly mild to moderate. In the EECOH-1 diabetes trial, reduced appetite affected 21.7% to 26.8% of people on the medicine (4.2% on placebo), diarrhoea 11.3% to 24.6% (5.6% on placebo) and nausea 7.2% to 12.7% (9.9% on placebo). These effects were most frequent while the dose was being increased and eased over time.
Overall: in the SLIMMER weight trial, 93% of people on the medicine and 84% on placebo reported at least one side effect, mostly digestive, and only 10 of the 499 people treated with it stopped because of side effects. Safety in pregnancy, breastfeeding and children has not been established in the published trials, so tell your doctor if you are pregnant, planning a pregnancy or breastfeeding.
