What Is Ganagliflozin Used For?

Ganagliflozin is a once-a-day tablet for type 2 diabetes, developed in China by Huisheng Biopharmaceutical, part of the Sihuan Pharmaceutical group, where it is sold as 惠优静® (Huiyoujing). It lowers blood sugar by helping the kidneys pass extra glucose out in the urine. In medical journals it is often spelled janagliflozin.

China's National Medical Products Administration (NMPA) approved it in January 2024 as a Class 1 innovative drug, to improve blood sugar control in adults with type 2 diabetes:

  • On its own, alongside diet and exercise
  • Together with metformin, when metformin alone does not control blood sugar

It is not for type 1 diabetes or diabetic ketoacidosis. It is a prescription diabetes and hormone medicine and should be used under an endocrinologist, diabetologist or physician.

How It Works

Normally the kidneys filter sugar out of the blood and then take almost all of it back in, using a transporter called SGLT2. Ganagliflozin blocks SGLT2, so the kidneys reabsorb less glucose and more of it leaves the body in the urine. Because this does not depend on insulin, it adds to the effect of other diabetes treatments.

What the Trial Showed

Two phase 3 trials in China, published in Diabetes, Obesity and Metabolism in 2023, support the approval. In 432 adults not controlled by diet and exercise, 24 weeks of treatment lowered HbA1c by 0.80% (25 mg) and 0.88% (50 mg) more than placebo, and about half reached an HbA1c below 7.0% (47.2% and 49.3%, against 23.5%). In 421 adults already taking metformin, adding it lowered HbA1c by 0.58% more than placebo at both doses. People also lost a little weight and saw small falls in systolic blood pressure, and no severe hypoglycaemia occurred over 52 weeks.

Recommended Dosage

  • Start with 25 mg once a day, before or after food
  • If more control is needed and 25 mg is well tolerated, the dose can go up to 50 mg once a day
  • Kidney function should be checked before starting and regularly afterwards; no dose change is needed for mild kidney impairment
  • Low body fluid (dehydration) should be corrected before the first dose

Your doctor sets the exact dose based on your blood sugar, kidney tests and other medicines.

Warnings and Precautions

Who should not take it: people allergic to the medicine or its ingredients, and people with severe kidney impairment, end-stage kidney disease or on dialysis. It is not recommended if eGFR stays below 30 mL/min/1.73 m².

  • Genital and urinary infections: because more sugar passes into the urine, genital yeast infections can occur. In women, they affected 4.4% on 50 mg against 1.0% on placebo. Urinary infections were not more common than with placebo (1.1% against 2.2%). Drink enough water and report burning, itching or discharge.
  • Ketoacidosis: no cases occurred in the trials, but this rare, serious condition has been reported with other medicines of the same class, sometimes with only mildly raised sugar. Seek help at once for nausea, vomiting, stomach pain, unusual tiredness or fast breathing. The doctor may pause it before surgery or during prolonged fasting.
  • Dehydration and low blood pressure: it works partly like a mild water tablet. Older people, people on diuretics and those with kidney problems need extra care.
  • Kidney function: a small, early dip in kidney filtration can occur. Kidney tests are recommended before starting and about every 6 months.
  • Feet: a higher risk of toe and foot amputation has been seen with other medicines of this class. Check your feet daily, especially if you have had ulcers or poor circulation.
  • Low blood sugar: rates were below 2% in the trials, with no severe episodes, but the risk rises when combined with insulin or tablets that push the pancreas to release insulin.

Other findings: ketones in the urine were seen in 10.0% (25 mg) and 14.1% (50 mg) against 4.3% on placebo, and raised bilirubin in 2.5% on 50 mg. LDL cholesterol rose slightly. It is not recommended during pregnancy or breastfeeding, and safety under 18 years has not been established.
