What Is Henagliflozin, Retagliptin and Metformin Used For?

This is a single extended-release tablet that combines three type 2 diabetes medicines: henagliflozin, retagliptin and metformin. It was developed in China by Jiangsu Hengrui Pharmaceuticals, its Chinese name is 恒格列净瑞格列汀二甲双胍缓释片, its brand name is 瑞乐唐®, and its research code was HR20031. Hengrui describes it as China's first home-grown three-in-one oral diabetes tablet. The idea is simple: fewer tablets to remember, which can make a long-term plan easier to follow.

China's National Medical Products Administration (NMPA) approved it in October 2025, as a prescription medicine (approval numbers 国药准字 H20250063 for strength I and H20250064 for strength II), for:

  • Adults with type 2 diabetes whose blood sugar is not well controlled on metformin
  • Used together with diet and exercise

It is a prescription endocrinology medicine and should be started and monitored by a diabetologist or endocrinologist.

How It Works

Each of the three ingredients lowers blood sugar in a different way:

  • Henagliflozin (an SGLT2 inhibitor) makes the kidneys pass extra sugar out in the urine.
  • Retagliptin (a DPP-4 inhibitor) keeps the gut hormones that trigger insulin release active for longer, so more insulin is released when blood sugar is high.
  • Metformin reduces the amount of sugar the liver releases and helps the body use insulin better.

What the Trial Showed

The key phase 3 trial, published in Diabetes, Obesity and Metabolism in 2024, included about 780 adults in China whose diabetes was not controlled on metformin. Everyone stayed on metformin and added retagliptin 100 mg, henagliflozin 5 mg or 10 mg, or retagliptin together with henagliflozin, once daily for 24 weeks.

  • With all three medicines together, HbA1c fell by 1.51% (henagliflozin 5 mg) and 1.54% (10 mg), compared with 0.86% to 0.98% when only one medicine was added.
  • About 57% of people on the triple approach reached an HbA1c below 7.0%, against 21% to 27% with a single add-on.
  • Fasting and after-meal sugar fell further, and the groups taking henagliflozin also lost more weight and had lower blood pressure.
  • Side effects were reported at similar rates in all groups, and there was no severe low blood sugar.

This trial used the three medicines as separate tablets; the combined tablet puts the same ingredients into one tablet.

Recommended Dosage

According to the Chinese prescribing information, it is usually taken once a day in the morning with a meal. The dose is chosen from the patient's current treatment, how well it is working and how well it is tolerated. Two strengths are approved (henagliflozin/retagliptin/metformin):

  • Strength (I), 5/50/750 mg: 2 tablets once a day
  • Strength (II), 10/100/1000 mg: 1 tablet once a day

The tablets are extended-release, so they should be swallowed as directed on the leaflet. Your doctor decides which strength suits you.

Warnings and Precautions

What the Prescribing Information Says

In the phase 3 study behind the approval, genital and urinary infections, a known concern with SGLT2 inhibitors, were reported in 1.9% of people, and no severe low blood sugar was reported. Side effects seen in at least 2% of people were weight loss (10.9%), raised lipase (7.1%), ketones in the urine (4.5%) and sugar in the urine (3.8%). Because the metformin is in extended-release form, stomach upset was uncommon: nausea 6.3%, vomiting 2.1% and diarrhoea 2.1%.

Cautions for Each Ingredient

Because the tablet contains metformin, an SGLT2 inhibitor and a DPP-4 inhibitor, the standard cautions for each of these medicine types apply, and your doctor will go through them with you:

  • Lactic acidosis (metformin): a rare but serious build-up of acid in the blood. The risk is higher with kidney or liver problems, heavy drinking, dehydration, surgery or X-ray dye scans. Get urgent help for unusual muscle pain, breathing difficulty, severe tiredness or stomach upset.
  • Ketoacidosis (SGLT2 inhibitors): can happen even when blood sugar is not very high. It is not for type 1 diabetes. Seek help for nausea, vomiting, stomach pain or unusual tiredness.
  • Genital and urinary infections (SGLT2 inhibitors): yeast infections and urinary infections are more likely. Rarely, a serious infection of the genital area can occur, so report pain, redness or swelling there with fever.
  • Dehydration and low blood pressure: extra sugar loss in the urine also draws out water. Drink enough fluids, especially in hot weather or during illness.
  • Pancreatitis and heart failure (DPP-4 inhibitors): stop and seek help for severe stomach pain spreading to the back; report breathlessness or ankle swelling.
  • Vitamin B12: long-term metformin can lower vitamin B12, so occasional blood tests are sensible.
  • Kidney function is checked before starting and regularly afterwards.

In the trial: side effects with the three medicines together were reported at rates similar to the single add-ons, and no episodes of severe low blood sugar were seen. The risk of low blood sugar can rise if it is combined with insulin or sulfonylureas. Safety in pregnancy, breastfeeding and children has not been established, so speak to your doctor first.

Documentation & Availability

What documents are required to import Henagliflozin, Retagliptin and Metformin to India?

The tablets can be imported only in the patient's name, by the patient or a government hospital, in line with India's import regulations for unapproved medicines. You will need:

  • A valid prescription from a qualified diabetologist, endocrinologist or physician, stating the strength
  • Recent reports such as HbA1c, fasting glucose and a kidney function test (creatinine/eGFR), with details of your current metformin dose
  • Patient ID proof (issued by the Government of India)

How is the order confirmed?

Once we have received and checked a valid prescription and, where needed, an import permit.

Is Henagliflozin, Retagliptin and Metformin available in India?

No. This combination is approved only in China. Neither henagliflozin nor retagliptin appears in CDSCO's lists of new drugs approved in India, the tablet is not sold in Indian pharmacies, and the US FDA and European Medicines Agency have not approved it either.

Patients can still get it legally through the Named Patient Program (NPP), which allows a medicine not registered in India to be imported for one specific patient on a prescription. Because it already contains metformin, your doctor will review your existing diabetes tablets before you switch.

It can be made available to patients, doctors, and hospitals across major cities in India, including Gurgaon, Delhi, Bangalore, Mumbai, Navi Mumbai, Thane, Hyderabad, Mysore, Kerala, Chennai, Coimbatore, Madurai, Kolkata, Ahmedabad, Surat, Vadodara, Pune, Nagpur, Nashik, Jaipur, Udaipur, Lucknow, Kanpur, Gorakhpur, Chandigarh, Mohali, Faridabad, Noida, Ghaziabad, Kochi, Trivandrum, Kozhikode, Thrissur, Kannur, Mangalore, Bhubaneswar, Patna, Ranchi, Raipur, Guwahati, Visakhapatnam, Vijayawada, Goa, Jammu, Srinagar, Dehradun, Bhopal, Indore, Ludhiana, Amritsar, and others — and can facilitate delivery to the patient's treating hospital. Orders are confirmed only after receipt of a valid doctor's prescription and an import permit, where applicable.
