What Is Prusogliptin Used For?

Prusogliptin is a once-a-day tablet for type 2 diabetes, developed in China by CSPC Pharmaceutical Group, where it is sold as 善泽平® (Shanzeping). It helps the body release insulin when blood sugar rises and keeps the sugar-raising hormone glucagon in check.

China's National Medical Products Administration (NMPA) approved it in January 2025 as a Class 1 new chemical drug, to improve blood sugar control in adults with type 2 diabetes:

  • On its own, alongside diet and exercise
  • Together with metformin, when metformin alone does not give adequate control

It is a prescription diabetes and hormone medicine and should be used under an endocrinologist, diabetologist or physician.

How It Works

Prusogliptin is a highly selective DPP-4 inhibitor. DPP-4 is an enzyme that quickly breaks down GLP-1, a gut hormone released after eating. By blocking the enzyme, prusogliptin keeps more active GLP-1 in the blood. This makes the pancreas more responsive to sugar: it releases more insulin when glucose is high and less glucagon, so blood sugar comes down.

What the Trial Showed

The approval was based on two phase 3 trials, one with prusogliptin on its own and one with metformin. The add-on trial, published in Diabetes, Obesity and Metabolism in 2022, included 214 adults in China whose diabetes was not controlled on metformin. After 24 weeks, HbA1c fell by 0.70% with prusogliptin plus metformin against 0.07% with placebo plus metformin, a difference of 0.63%. Half of the patients on prusogliptin (50.0%) reached an HbA1c of 7.0% or lower, compared with 21.1% on placebo, and after-meal sugar dropped by 2.43 mmol/L against 0.70 mmol/L.

Recommended Dosage

  • 100 mg (one tablet) once a day, whether used alone or with metformin
  • It can be taken with or without food
  • According to the company, no dose change is needed for mild to moderate kidney impairment

Your doctor will confirm the dose and how it fits with your other diabetes medicines.

Warnings and Precautions

Only for type 2 diabetes: it is approved to improve blood sugar control in type 2 diabetes. It is not a treatment for type 1 diabetes or diabetic ketoacidosis.

  • Low blood sugar: in the add-on trial no hypoglycaemia was judged to be caused by the medicine. The risk can rise if it is combined with insulin or tablets that push the pancreas to release insulin, so tell your doctor everything you take.
  • Liver tests: abnormal liver function was among the side effects linked to treatment, in 1.4% of patients, the same rate as with placebo. Your doctor may check liver tests during follow-up.
  • Sleep: trouble sleeping was reported as a treatment-related side effect in 1.4% of patients.
  • Kidneys: the company states that no dose change is needed for mild to moderate kidney impairment. People with severe kidney disease should discuss it with their doctor first.

Overall: in the 24-week add-on trial, side effects judged to be related to treatment occurred in 8.5% of people on prusogliptin and 11.3% on placebo. Serious side effects affected 4.2% against 5.6%, and only 1.4% in each group stopped because of side effects. Body weight did not change meaningfully. Safety in pregnancy, breastfeeding and children has not been established in the published data. Your doctor should read the full Chinese prescribing information for the complete warnings.
