Trikafta (elexacaftor/tezacaftor/ivacaftor) is a CFTR modulator combination approved by the FDA for the treatment of cystic fibrosis (CF) in patients aged 2 years and older who have at least one F508del mutation in the CFTR gene, or another mutation responsive to the combination based on clinical and in vitro data. Trikafta is the US brand of the same active ingredients marketed in the EU and UK as Kaftrio.
Trikafta contains three CFTR modulators that work together: elexacaftor and tezacaftor help the defective CFTR protein fold and traffic correctly to the cell surface, while ivacaftor helps the CFTR protein open more effectively to allow chloride ion transport, addressing the underlying protein defect rather than only managing symptoms.
Trikafta's label has been progressively expanded to cover an increasing range of CFTR variants beyond F508del, based on clinical and laboratory data — a treating CF specialist can confirm current eligibility based on a patient's specific genotype.
For adults and adolescents 12 years and older, the standard regimen is two tablets (elexacaftor 100 mg/tezacaftor 50 mg/ivacaftor 75 mg) in the morning and one ivacaftor 150 mg tablet in the evening, approximately 12 hours apart, taken with fat-containing food. Pediatric dosing uses age- and weight-appropriate tablet or granule formulations.
Boxed Warning — Trikafta: Trikafta can cause serious and potentially fatal drug-induced liver injury. Cases of liver failure leading to transplantation and death have been reported in patients with and without a history of liver disease taking Trikafta, in both clinical trials and the postmarketing setting.
Liver function tests are recommended at baseline, then monthly for the first six months, then every three months for the next twelve months, and annually thereafter. Trikafta should be interrupted if signs of significant liver injury occur.
Hypersensitivity reactions, including anaphylaxis and angioedema, have been reported.
Cases of intracranial hypertension have been reported, and patients should be advised to report symptoms such as headache, vision changes, or nausea.
Mood and behavioral changes have been reported, particularly in adolescents. Non-congenital cataracts have been reported in pediatric patients; ophthalmological exams are recommended before and during treatment in this population.

Trikafta is manufactured and marketed by Vertex Pharmaceuticals Incorporated in the United States. It is not separately marketed in the EU or UK — the identical elexacaftor/tezacaftor/ivacaftor combination is sold there under the brand name Kaftrio. NPP sources Trikafta through authorized US distribution channels for patients seeking named-patient import into India.
Patients typically need a valid prescription from a treating pulmonologist or CF specialist confirming CFTR genotype and eligibility, along with supporting clinical records. NPP's regulatory team assists in preparing and verifying the documentation needed for a compliant import.
As an FDA-approved therapy for a rare genetic disease, Trikafta is sourced through authorized US distribution channels and imported under GDP-compliant logistics to preserve product quality throughout transit.
NPP sources Trikafta directly from authorized US pharmaceutical distributors to ensure product authenticity and full traceability. Every shipment is handled under Good Distribution Practice (GDP) conditions from origin to the patient's doorstep.
Trikafta should be stored at 20°C to 25°C, with excursions permitted between 15°C and 30°C, and this condition is maintained throughout transit.
Once documentation and regulatory approvals are in place, delivery timelines are communicated transparently at the time of quotation, accounting for import clearance and last-mile logistics to the patient's city.
