What Is Alfosbuvir Used For?

Alfosbuvir is an oral antiviral tablet for chronic hepatitis C, developed and made in China by Nanjing Sanhome Pharmaceutical, where it is sold as 圣诺迪® (Sanoldi®). It is always taken together with a second antiviral, daclatasvir. The two attack the hepatitis C virus at different points, and a 12-week course is aimed at clearing the virus from the body.

China's National Medical Products Administration (NMPA) approved it in May 2023, through its priority review route, for adults with:

  • Chronic hepatitis C virus (HCV) infection of genotypes 1, 2, 3 or 6
  • Who have never been treated, or who were treated before with interferon
  • With or without compensated cirrhosis (scarred liver that is still working)

It is a prescription liver medicine and should be used under a hepatologist or gastroenterologist.

How It Works

The hepatitis C virus copies itself using an enzyme called NS5B polymerase. Alfosbuvir blocks that enzyme, while daclatasvir blocks a different viral protein, NS5A. Hitting the virus in two places at once makes it very hard for it to keep multiplying.

What the Trial Showed

In the phase 3 study published in Infectious Diseases and Therapy in 2023, 326 adults in China took alfosbuvir with daclatasvir for 12 weeks. 98.2% were cured, meaning no virus could be found 12 weeks after the last tablet (SVR12). The cure rate was 99.3% in people without cirrhosis and 92.7% in those with compensated cirrhosis. It worked across genotypes 1, 2, 3 and 6, including 100% of the 95 patients with genotype 2a.

Recommended Dosage

  • 600 mg (six 100 mg tablets) once a day, with or without food
  • Taken together with daclatasvir 60 mg (one tablet) once a day
  • Both for 12 weeks; the label says the total course must not go beyond 12 weeks
  • No dose change is needed for mild or moderate liver impairment (Child-Pugh A or B)

The dose should not be reduced, and it is never taken on its own. If daclatasvir is stopped, alfosbuvir is stopped too.

This information is not a treatment plan. For personalized medical advice, consult your doctor and refer to the full prescribing information.

Warnings and Precautions

Never used alone: it only works as part of a two-drug combination. Read the prescribing information of daclatasvir as well, since its own warnings and drug interactions also apply.

  • 12 weeks maximum: animal studies on long-term cancer risk had not been completed at approval, so the label does not allow a course longer than 12 weeks.
  • Severe liver disease: safety and effectiveness have not been established in people with decompensated (Child-Pugh C) liver disease.
  • Kidney problems: it has not been studied in people with kidney impairment, so the doctor will weigh this carefully.
  • Cholesterol and blood fats: raised cholesterol was the most common side effect in the trial (13.2%), along with high blood lipids (6.4%) and triglycerides (5.8%). Your doctor may check these.
  • Hepatitis B: as with any direct-acting hepatitis C treatment, doctors usually test for hepatitis B first, because a hidden hepatitis B infection can flare once hepatitis C is cleared.
  • Other medicines: give your doctor a full list of what you take, including herbal products, because some medicines interact with daclatasvir.

Side effects in the trial: most were mild. 7.7% of patients had a serious health event during the study, but none was judged to be caused by the medicines. Only one patient (0.3%) stopped treatment, because of liver cancer, and no one died. Safety in pregnancy and breastfeeding has not been established in the published data, so discuss this with your doctor first.

Documentation & Availability

What documents are required to import Alfosbuvir to India?

The tablets can be imported only in the patient's name, by the patient or a government hospital, in line with India's import regulations for unapproved medicines. You will need:

  • A valid prescription from a qualified hepatologist or gastroenterologist
  • Diagnostic reports, including HCV RNA (viral load), the HCV genotype and a liver assessment such as FibroScan or ultrasound
  • Patient ID proof (issued by the Government of India)

How is the order confirmed?

Once we have received and checked a valid prescription and, where needed, an import permit.

Is Alfosbuvir available in India?

No. It is approved only in China. It has no CDSCO approval, it is not sold in Indian pharmacies, and the US FDA and European Medicines Agency have not approved it either.

Patients can still get it legally through the Named Patient Program (NPP), which allows a medicine not registered in India to be imported for one specific patient on a prescription. Because it is always taken with daclatasvir, your doctor will plan both medicines together.

It can be made available to patients, doctors, and hospitals across major cities in India, including Gurgaon, Delhi, Bangalore, Mumbai, Navi Mumbai, Thane, Hyderabad, Mysore, Kerala, Chennai, Coimbatore, Madurai, Kolkata, Ahmedabad, Surat, Vadodara, Pune, Nagpur, Nashik, Jaipur, Udaipur, Lucknow, Kanpur, Gorakhpur, Chandigarh, Mohali, Faridabad, Noida, Ghaziabad, Kochi, Trivandrum, Kozhikode, Thrissur, Kannur, Mangalore, Bhubaneswar, Patna, Ranchi, Raipur, Guwahati, Visakhapatnam, Vijayawada, Goa, Jammu, Srinagar, Dehradun, Bhopal, Indore, Ludhiana, Amritsar, and others — and can facilitate delivery to the patient's treating hospital. Orders are confirmed only after receipt of a valid doctor's prescription and an import permit, where applicable.

Sourcing & Delivery

NamedPatientProgram.com (NPP) sources this medicine through the manufacturer's authorised supply channels in China and arranges supply to patients, doctors, and hospitals under applicable legal provisions.
