Understanding Mekinist (Trametinib) and Its Medical Uses

Mekinist (trametinib) is an oral targeted therapy that inhibits MEK1 and MEK2, key proteins in the MAPK signaling pathway that drives tumor growth in cancers with a BRAF V600 mutation. It is often used together with Tafinlar (dabrafenib) for enhanced effect.

Approved Indications

Mekinist is FDA-approved, as monotherapy or in combination with dabrafenib, for adult patients with unresectable or metastatic melanoma carrying a BRAF V600 mutation. In combination with dabrafenib, it is also approved for adjuvant treatment of resected Stage III BRAF V600-mutant melanoma, for BRAF V600-mutant advanced non-small cell lung cancer, and for BRAF V600E-mutant anaplastic thyroid cancer.

How It Works

By blocking MEK1/MEK2, trametinib interrupts the MAPK signaling cascade downstream of mutated BRAF, slowing or halting tumor cell growth.

Dosing

The recommended dose is 2 mg orally once daily, taken at least 1 hour before or 2 hours after a meal, either alone or in combination with dabrafenib (150 mg twice daily), as determined by the treating oncologist.

Warnings and Precautions for Mekinist

Mekinist requires monitoring for several serious adverse effects, particularly when combined with dabrafenib.

Cardiomyopathy

Assess left ventricular ejection fraction before starting, at one month, and periodically during treatment.

Ocular Toxicity

Retinal vein occlusion and retinal pigment epithelial detachment have been reported. Perform ophthalmologic exams periodically and for any visual disturbances.

Interstitial Lung Disease / Pneumonitis

Monitor for new or worsening pulmonary symptoms; withhold or discontinue as needed.

Serious Skin Toxicity

Rash and other skin reactions can occur, including severe cases; monitor closely, especially in combination therapy.

Hemorrhage

Increased bleeding risk, particularly when combined with dabrafenib. Monitor for signs of bleeding.

Hyperglycemia

Monitor blood glucose, particularly in diabetic patients.

Embryo-Fetal Toxicity

Mekinist can cause fetal harm. Advise effective contraception during treatment and for a defined period after the final dose.

  • Cardiac and ophthalmologic monitoring is required throughout treatment.
  • Report any new visual changes, breathing difficulty, or skin reactions promptly.
  • Dosing and monitoring intervals differ between monotherapy and combination therapy — follow the treating oncologist's regimen.
