What Is Simnotrelvir Used For?

Simnotrelvir is an oral antiviral for COVID-19, developed by Simcere Pharmaceutical with the Shanghai Institute of Materia Medica and the Wuhan Institute of Virology, and sold in China as 先诺欣® (Xiannuoxin). It comes as a combination pack of simnotrelvir tablets and low-dose ritonavir tablets, which are swallowed together for five days.

China's National Medical Products Administration (NMPA) first approved it with conditions on 28 January 2023 and converted this to a regular (full) approval in July 2024. It is approved for adults with:

  • Mild to moderate COVID-19 (SARS-CoV-2 infection)
  • Started as soon as possible, within 3 days of the first symptoms

As the virus keeps changing, the label notes that its effect against newer strains needs to be kept under study. It is a prescription infection medicine and should only be taken on a doctor's advice.

How It Works

The coronavirus needs an enzyme called the 3CL (main) protease to cut its proteins into working parts before it can make copies of itself. Simnotrelvir blocks that enzyme, so the virus cannot multiply. Low-dose ritonavir slows the breakdown of simnotrelvir in the body, which keeps its blood level high enough to work.

What the Trial Showed

The phase 2/3 trial, published in the New England Journal of Medicine in 2024, included 1,208 adults with mild to moderate COVID-19 at 35 sites in China. Almost all were vaccinated, and about half had at least one risk factor for severe illness. In people who started within 72 hours of symptoms, all 11 tracked symptoms cleared about a day and a half sooner than with placebo (median 180.1 vs 216.0 hours). By day 5, the viral load had fallen by an extra 1.51 log10 copies/mL. No one in either group developed severe COVID-19 or died, so the trial could not measure an effect on serious illness.

Recommended Dosage

  • Simnotrelvir: 750 mg (2 pink tablets of 375 mg)
  • Ritonavir: 100 mg (1 white tablet), taken at the same time
  • Every 12 hours, for 5 days in a row
  • On an empty stomach, swallowed whole: do not chew, split or crush the tablets

Simnotrelvir must always be taken with ritonavir. No dose change is needed for older adults or for mild to moderate liver impairment. For moderate kidney impairment (eGFR 30 to below 60 mL/min) the dose is lowered to 1 simnotrelvir tablet with 1 ritonavir tablet every 12 hours. It should not be used with severe liver or kidney impairment (eGFR below 30), including dialysis.

This information is not a treatment plan. For personalized medical advice, consult your doctor and refer to the full prescribing information.

Warnings and Precautions

Drug interactions (most important): the ritonavir in the pack strongly blocks the liver enzyme CYP3A. This can push up the levels of many everyday medicines, including some heart, blood-pressure, cholesterol, pain and sedative medicines, sometimes to dangerous levels. Some other medicines and herbal remedies can lower simnotrelvir levels and stop it working. The Chinese leaflet lists medicines that must not be combined with it, so give your doctor a full list of everything you take before starting.

  • Pregnancy: it must not be used during pregnancy. Women who could become pregnant should use effective contraception during treatment and for 7 days afterwards. Ritonavir can weaken hormonal birth control, so a barrier method is advised until one menstrual cycle after the course.
  • Breastfeeding: stop breastfeeding during treatment and for 7 days after the last dose.
  • Liver: ritonavir has been linked to raised liver enzymes, hepatitis and jaundice, so extra care is needed with liver disease. Raised liver enzymes (ALT and AST) have also been reported since launch.
  • HIV: in people with untreated or undiagnosed HIV, the ritonavir may lead to HIV drug resistance.
  • Lactose: the simnotrelvir tablet contains lactose, so it should not be used by people with rare inherited lactose or galactose problems.
  • Common side effects: a fall in neutrophil count, raised blood fats, diarrhoea, nausea, itching and a slow heartbeat. The neutrophil fall never went below 1.0 × 10⁹/L in the trial.

Overall: in the trial, side effects were reported in 29.0% of people on simnotrelvir and 21.6% on placebo, mostly mild or moderate. The most frequent drug-related ones were a rise in triglycerides (4.3% vs 2.1%), a fall in neutrophils (1.9% vs 0.2%) and diarrhoea (1.7% vs 1.1%). There were no serious side effects in the simnotrelvir group, and 1.2% stopped treatment. It has not been tested in children under 18.

Documentation & Availability

What documents are required to import Simnotrelvir to India?

The tablets can be imported only in the patient's name, by the patient or a government hospital, in line with India's import regulations for unapproved medicines. You will need:

  • A valid prescription from a qualified physician, infectious disease specialist or pulmonologist
  • A positive COVID-19 test report (RT-PCR or antigen) and the date symptoms began
  • A recent kidney function result (creatinine or eGFR) and a list of current medicines
  • Patient ID proof (issued by the Government of India)

How is the order confirmed?

Once we have received and checked a valid prescription and, where needed, an import permit. Since treatment should start within 3 days of the first symptoms, please contact us as early as possible.

Is Simnotrelvir available in India?

No. It is approved only in China. It has no CDSCO approval, it is not sold in Indian pharmacies, and the US FDA and European Medicines Agency have not approved it either.

Patients can still get it legally through the Named Patient Program (NPP), which allows a medicine not registered in India to be imported for one specific patient on a prescription. NPP also arranges other antiviral medicines that are not sold in India, such as Livtencity.

It can be made available to patients, doctors, and hospitals across major cities in India, including Gurgaon, Delhi, Bangalore, Mumbai, Navi Mumbai, Thane, Hyderabad, Mysore, Kerala, Chennai, Coimbatore, Madurai, Kolkata, Ahmedabad, Surat, Vadodara, Pune, Nagpur, Nashik, Jaipur, Udaipur, Lucknow, Kanpur, Gorakhpur, Chandigarh, Mohali, Faridabad, Noida, Ghaziabad, Kochi, Trivandrum, Kozhikode, Thrissur, Kannur, Mangalore, Bhubaneswar, Patna, Ranchi, Raipur, Guwahati, Visakhapatnam, Vijayawada, Goa, Jammu, Srinagar, Dehradun, Bhopal, Indore, Ludhiana, Amritsar, and others — and can facilitate delivery to the patient's treating hospital. Orders are confirmed only after receipt of a valid doctor's prescription and an import permit, where applicable.

Sourcing & Delivery

NamedPatientProgram.com (NPP) sources this medicine through the manufacturer's authorised supply channels in China and arranges supply to patients, doctors, and hospitals under applicable legal provisions. Each box is one full 5-day course: five blister strips, one per day, each holding 4 simnotrelvir and 2 ritonavir tablets. The label asks for the pack to be kept sealed at no more than 30°C, so it travels in its original packaging under temperature-controlled shipping. All prescriptions are dispensed and checked by registered pharmacists and dispatched from New Delhi, India, after legal requirements are fulfilled.
